The Evertz HD Caption Encoder (HD9084) along with whichever captioning software you're comfortable with is one way.   The good thing with the Evertz is that it does both HD (708) and SD (608) captioning simultaneously or can transcode from one to the other.   I've used it in the past and it does a really great job.
