I've solved it... I first tried with the BSP settings, then map
conversion, nothing worked. Then last night I was talking to a friend
that works at another studio (maya/mental ray based) and he said he
have had the same problems. He then suggested that I try and render it
with only one thread activated. Using SMP can sometimes cause these
artifacts and generate no errors.
Using only one thread helped a bit, then also switching mental ray to
use the tiling "spiral" instead if "hilbert" solved the last few
chunks that was not rendered (I didn't know the tiling could also mess
with the rendering).

So now I've successfully rendered a 16-bit Tif file and also a 16bit
SGI file with a normal map also generated.
