I've also run into this problem since XSI 5.0, but rather intermittantly. Too rarely to ever lock down specific repro steps and report it to Soft. One person's CAB's alone won't be enough to figure this out!
I think what we need to do to help nip this one is the moment any of realizes the problem is happening, we have to create CABs files and send them to soft. The collection of a handful of CABs should have enough of a cross section where the common cause can be determined.
I can volunteer to be point man on this, so I can get a specific bug log number everyone can use to reference this.
