are you sure you didn't do something like manipulating points in shape modelling mode?
I've had similar oddities when I, unaware, had stuff going on at entry points other then modelling, and then when adding ops that would change the connectivity scheme (that are automatically put in the modelling stack) got really odd results.
it's a consequence of a nice design element, but from time to time it can leave you puzzled for a while before you notice it was the user (me in my case) being a dolt.
