Updating an existing custom property requires special handling.

The newer version of your plug-in (the one with the new toggle) has to check if the toggle is missing, and if it is missing, add it. See here for a crude example:

A new version of a plug-in can also update existing parameters with the EditParameterDefinition command.

For me even saving the scene and reloading won't introduce the new
parameters. The only way I've found is to trash the property and re-apply
it. I have one right here that's being a pain on scenes with a property
applied before the plugin was modified to include an extra toggle. Toggle
doesn't appear in the older scenes at all.
