Subdivide and Displace Plugin

Anyone know how to use this?

I last used it about 2 years ago on a job, awesome
tool, and at the moment a freelancer I am working with
could really use it for something he's doing.  Problem
is, I cannot recall exactly how to use it, and there
are no instructions on Helge's site.

I cant quite recall how to apply the image map for it
to use for sub-division.  I've tried a texture map,
and an image map, but it's unclear what it needs to
work.
