i would recommend using tokens, they streamline the process extremely well.
if you are can include an example of your pass output tokens?
if you are trying to type an absolute path into the pass output, keep in mid it is added to the scene output (turn on resolved path to check)
so for the path options just type in the chunk after the scene options.
ie:
scene options: c:\temp\
pass options: [pass]\[pass]_[framebuffer].[frame].exr
results in: c:\temp\reflection\reflection_main.0001.exr
if tokens make you nauseous this ought to do the same thing
pass options: reflection\reflection_main.###.exr
the only thing i can imagine you are doing wrong is typing in the whole pass path into the pass output
