That's those damn NAN value artifacts.  There are a couple NAN busters out there.  You can try Halfdan's Nass shader at:  http://www.vol2nuit.fr/v2blog/antiNaNs.rar .  There's also an antiNaNs shader I believe. I think its different from Halfdan's, not sure though.  I can't find it on the web but I've attached it as a zip file.  Let me know if it doesn't come through.

I'm pretty sure that has nothing to do with scene complexity.

On this project... http://www.genecrucean.com/vids/pdfa_shrinking.html I had those black spots quite a bit on the football. Geometry wise this was a very simple scene. What was causing it in this situation was some funky geometry on the football laces. Our black spots were localized to the laces so maybe that will help you find the problem a little faster in a heavy scene.

Somehow I got rid of the problem. What precisely solved it, I don't
know. But now I have new one:

http://www.auner.net/misc/landscape_pressbaum_Default_Pass_3e_crop.jpg

This is a huge scene, the FG parameters are really high. If I lower
them, the whole things becomes spotty and uneven. But now I get these
strange artifacts, in the squares in either black or in yellow.
Anybody got a solution?

Thanks in advance!
