I've had that many times. Not everyday, but on numerous ocassions.
Thankfully, XSI lets me save the scene just fine, but it gets very
glitchy and can crash very easily if you don't close it down when the
bug happens. No scene corruption, so as Costa says, it's a minor
glitch.
I personally haven't figured out how to reproduce it yet... it just
happens... when renaming something in the Explorer, sometimes. :/

ps: 6.02 here, but I believe I recall it happening in 6.0 too. (I
don't remember if I had the bug back with 5.11, though.)
